A less common precipitant to AHF is one of a several possible non-ischemia myocardial insults. This includes the onset of acute myocardial dysfunction with inflammatory insults, toxic insults, and insults of an undefined nature such as peripartum cardiomyopathy. Admission to hospital with AHF can often herald a diagnosis of chronic HF as these acute insults may have long-term sequelae on myocardial function. Acute HF severity can be measured using known predictors of poor outcome, such as hypotension, renal dysfunction, hyponatremia, elevated biomarkers, respiratory distress, or concomitant comorbidities. The large majority of patients presenting with AHF do so in the context of pre-existing cardiomyopathy, a situation described as acute decompensated heart failure (ADHF). There are several key differences between this group of patients and those presenting with de novo AHF. Unlike with de novo AHF, patients with ADHF tend to present with signs and symptoms of congestion and fluid retention (weight gain, exertional dyspnea, orthopnea, dependent edema) rather than with pulmonary edema or cardiogenic shock that characterize acute LV systolic dysfunction.

  • The study conducted by Farmakis et al. (2015) found that the patients admitted for AHF were aged >70 years, and about half of them are male. Most have a previous history a heart failure, while de novo AHFrepresents only one-fourth to one-third of cases. About 40–55% have preserved LVEF (left ventricular ejection fraction).
  • In terms of noncardiovascular comorbidities, about 40% of patients admitted for AHF have a history of diabetes mellitus, about one-fourth to one-third have renal dysfunction and chronic obstructive pulmonary disease, while anemia was also present in 15–30% of patients. Bouee et al. (2014) described the incidence and profile of patients hospitalized for acute heart failure (AHF) and found that incidence rates were 2.28‰ in 2006 and 2.45‰ in 2010. The proportion of patients aged ≥75 increased from 71.0% in 2006 to 74.3% in 2010. Half of the patients were male.   • In a study by Tavazzi et al. (2006), it was found that out of total selected patients, 44% patients were diagnosed as having de novo acute HF, and 56% had worsening chronic HF. An ejection fraction (EF)>40% was found in 34% of cases.
  • The study by Kurmani et al. (2017) found that 20–30% of patients with ADHF had an element of renal dysfunction, and 40% have diabetes mellitus.
